We understand the importance of your time and will do our best to meet you when you are scheduled to be there. We appreciate your patience in the event that delays occur due to unforeseeable circumstances. If you’re likely to be late to your appointment, please contact us before you arrive, as it is possible that we will need to change your appointment. If you have to alter or cancel your appointment, please contact us at minimum 24 hours prior to the appointment so that another patient to use the time slot on the schedule.
Click on the appropriate forms below. The forms include an Patients Questionnaire as well as HIPPA Recognized and Financial Responsibility forms, as and a records transfer form. Bring the complete forms (unless otherwise specified) for your visit. Also bring:
- The current insure card(s)
- Driver’s License
